A resolution approving amendments two and three to a grant from the U.S. Department of Health and Human Services to the Metropolitan Government, acting by and through the Metropolitan Board of Health, to enhance access to a comprehensive continuum of high quality, community-based care for low-income individuals and families with HIV disease.

WHEREAS, the Metropolitan Government, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the U.S. Department of Health and Human Services to enhance access to a comprehensive continuum of high quality, community-based care for low-income individuals and families with HIV disease approved by RS2020-376; and,
WHEREAS, amendment two to the grant allows the grantee to submit a revised form SF 424A Budget Narrative Justification and Service Category Plan Table for the total award of $4,374,504.00, a copy of which amendment two is attached hereto; and,
WHEREAS, amendment three de-obligates $426,692.00 from the previous grant award of $4,374,504.00 for a new total of $3,947,812.00, a copy of which amendment three is attached hereto; and,
WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendments two and three be approved.
NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:
Section 1. That amendments two and three to the grant by and between the U.S. Department of Health and Human Services and the Metropolitan Government, acting by and through the Metropolitan Board of Health, to enhance access to comprehensive high quality, community-based care for low-income individuals and families with HIV disease, copies of which amendments two and three are attached hereto and incorporated herein, are hereby approved, and the Metropolitan Mayor is hereby authorized to execute the same.
