A resolution approving Amendment Number 2 to a sole source contract between the Metropolitan Government of Nashville and Davidson County and The Village at Glencliff for the provision of homelessness medical respite services.

WHEREAS, The Metropolitan Government of Nashville and Davidson County ("Metro") previously entered into Contract 6563187, with The Village at Glencliff to provide homelessness medical respite services; and,
WHEREAS, the parties previously amended the contract via Amendment Number 1 to the contract to extend the term and increase the value of the original contract; and,
WHEREAS, the parties desire to enter into Amendment Number 2 to this contract to extend the term of the contract, and to increase the value, to remove the requirement to identify the project name, RFQ or Contract number on the ACORD document; and,
WHEREAS, approval of the contract amendment attached hereto and incorporated herein with The Village at Glencliff. will benefit the citizens of the Metropolitan Government of Nashville and Davidson County.
NOW THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:
Section 1. That Amendment Number 2 to a contract between The Metropolitan Government of Nashville and Davidson County and The Village at Glencliff (contract number 6563187), to extend the term of the contract and to increase the value, to remove the requirement to identify the project name, RFQ or Contract number on the ACORD document, a copy of which is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.
Section 2. That this Resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
