WHEREAS, the Metropolitan Government, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the Centers for Disease Control and Prevention to address social determinants of health to improve chronic disease outcomes among persons experiencing health disparities and inequities approved by RS2021-1244; and,

WHEREAS, the parties wish to amend the grant agreement to extend the end date of the grant from September 29, 2022 to March 29, 2023, a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be approved. 

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1.                     That amendment one to the Nashville Health Accelerator Plan grant by and between the Centers for Disease Control and Prevention and the Metropolitan Government, acting by and through the Metropolitan Board of Health, to address social determinants of health to improve chronic disease outcomes among persons experiencing health disparities and inequities, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2.                     That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.

This resolution approves the first amendment to a Nashville Health Accelerator Plan grant from the Centers for Disease Control and Prevention and the Metropolitan Board of Health, originally approved by Resolution No. RS2021-1244.

 

The grant proceeds are used to address social determinants of health to improve chronic disease outcomes among persons experiencing health disparities and inequities. The grant award is $125,000.

 

The amendment under consideration extends the end date of the grant from September 29, 2022, to March 29, 2023. No other changes would be made to the grant agreement.
