WHEREAS, the Metropolitan Government, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the Tennessee Department of Health, to improve the health of those residing in or visiting Davidson County through targeted strategies to prevent and control the use of tobacco products approved by RS2018-1305; and,

WHEREAS, the parties wish to amend the grant agreement to increase the amount of the grant by $10,000.00 from $619,900.00 to $629,900.00 and to extend the end date of the grant contract to June 30, 2023, a copy of which amendment six is attached hereto; and,  

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment six be accepted.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1.                     That amendment six to the grant by and between the Tennessee Department of Health and the Metropolitan Government, acting by and through the Metropolitan Board of Health, to improve the health of those residing in or visiting Davidson County through targeted strategies to prevent and control the use of tobacco products, a copy of which amendment six is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2.                     That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.

This resolution approves the sixth amendment to a grant from the Tennessee Department of Health to the Metropolitan Board of Health The grant, previously approved by RS2018-1305, was to improve the health of those residing in or visiting Davidson County through targeted strategies to prevent and control the use of tobacco products. The amendment increases the grant amount by $10,000 from $619,900 to $629,900. The end date would be extended from June 30, 2022, to June 30, 2023.
