WHEREAS, the Metropolitan Government, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the State of Tennessee, Department of Health, to facilitate the planning, implementation, and evaluation of community-driven and evidence-based health promotion programs approved by resolution RS2020-185; and,
WHEREAS, the parties wish to amend the grant agreement to increase the amount of the grant by $400,000.00 from $318,500.00 to $718,500.00 and to extend the end date to June 30, 2024, a copy of which amendment one is attached hereto; and,
WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be approved.
NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:
Section 1. That amendment one to the grant by and between the State of Tennessee, Department of Health, and the Metropolitan Government, acting by and through the Metropolitan Board of Health, to facilitate the planning, implementation, and evaluation of community-driven and evidence-based health promotion programs, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.
Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.

This resolution approves the first amendment to a grant between the Tennessee Department of Health and the Metro Board of Health. Resolution No. RS2020-185 accepted a grant to facilitate the planning, implementation, and evaluation of community-driven and evidence-based Health Promotion programs.
This amendment would increase the grant amount by $400,000, from $318,500 to $718,500. The end date would be extended from June 30, 2022 to June 30, 2024. New grant budget attachments would be added to the agreement. No other changes would be made to the grant agreement.
