WHEREAS, the Metropolitan Government, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the State of Tennessee, Department of Human Services, to conduct immunization record audits for child-care centers, drop-in centers, and group child-care homes to ensure the safety and well-being of children and families in Tennessee approved by RS2024-713; and, 

WHEREAS, the parties wish to amend the grant agreement to increase the amount of the grant by $152,000.00 from $152,000.00 to $304,000.00 with no cash match required, extend the grant end date to September 30, 2026, and update Term E.2. Communications and Contacts; and, 

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be approved.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1.                     That amendment one to the grant by and between the State of Tennessee, Department of Human Services, and the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to conduct immunization record audits for child-care centers, drop-in centers, and group child-care homes to ensure the safety and well-being of children and families in Tennessee, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2.                     That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.

The resolution approves the first amendment to a grant agreement from the Tennessee Department of Human Services to the Metropolitan Board of Health (“Metro Health”), as approved by Resolution No. RS2024-713. The grant provides for Metro Health to conduct immunization record audits for child care centers, drop-in centers, and group child care homes to ensure the safety and well-being of children and families in Tennessee.

 

The proposed amendment would increase the amount of the grant by $152,000 from $152,000 to $304,000 with no cash match required. The term of the grant would also be extended to September 30, 2026, with options for two 12-month renewals. The amendment would also update contact information for the parties.
